Consultation has concluded

Social Prescribing connects older adults (55+) to non-medical community supports to improve overall well-being. Through a referral process, individuals receive assistance from a Case Manager and Link Worker who helps identify social, emotional and non-medical needs and connects them to appropriate services.

Social prescribing assists with:

  • Social isolation or loneliness
  • Mental health challenges (e.g., stress, anxiety, mild depression)
  • Difficulty accessing community resources
  • A need for support with non-medical aspects of daily living

The link worker helps individuals connect with:

  • Social groups, clubs, and community activities
  • Transportation services
  • Meal programs
  • In-Home support (basic light housekeeping, meal preparation)
  • Health and wellness programs (e.g., fitness, mental health support)
  • Financial and housing resources
  • Connection to Council on Aging for their lawn maintenance and snow removal program
