Social Prescribing Information Sessions
Consultation has concluded
Social Prescribing connects older adults (55+) to non-medical community supports to improve overall well-being. Through a referral process, individuals receive assistance from a Case Manager and Link Worker who helps identify social, emotional and non-medical needs and connects them to appropriate services.
Social prescribing assists with:
- Social isolation or loneliness
- Mental health challenges (e.g., stress, anxiety, mild depression)
- Difficulty accessing community resources
- A need for support with non-medical aspects of daily living
The link worker helps individuals connect with:
- Social groups, clubs, and community activities
- Transportation services
- Meal programs
- In-Home support (basic light housekeeping, meal preparation)
- Health and wellness programs (e.g., fitness, mental health support)
- Financial and housing resources
- Connection to Council on Aging for their lawn maintenance and snow removal program
